Welcome to Creekside Christian Academy and thank you for your interest in our school.. As you consider placing your student at Creekside Christian Academy there are a few ways you can begin the process.


Below you will find information on how to go about getting basic information, setting up a tour, filling out the application and then executing the admission process.

How to Apply


Begin by completing the ONLINE APPLICATION. Click on the button below to fill out and application. You will be redirected to a secure website where you will input all of student's information and pay the Application Fee ($250.00 for K5-12th Grades; $175 for PK3-PK4). 


Note: If a student is the recipient of IEP services please remit a copy to the admission director prior to submitting and application.

Documents


  • The MINISTRY REFERRAL should be completed and mailed or faxed to the CCA Admissions Director.
     
  • We request that the student’s current teacher complete the enclosed TEACHER RECOMMENDATION form and mail or fax it to CCA. For elementary students, the primary teacher should fill out the recommendation. Students entering 7th-12th grades need to have two Teacher Recommendations from their Math and English teachers. Preschool students should have a form completed if they have previously attended a Preschool program or day care.
     
  • Turn in the AUTHORIZATION TO RELEASE STUDENT RECORDS to the student’s current school and have them send transcripts and test scores to CCA. It is the applicant’s responsibility to make sure that the transcripts and test grades are received by the CCA.
     
  • Birth Certificate, Immunization and ear/eye/dental evaluations need to be sent in to complete the students file.)


Testing


Admission Testing is conducted online or we may have you come in to the school for testing. All 1-12th students are tested. If they are coming from a school that conducts CogAt, Terranove, PSAT, IOWA or ITBS, and the results are less than a year old, we will not need to conduct testing. 

Interview


After all requested documents have been received, the Admission Committee will determine whether to grant an INTERVIEW. If an interview is scheduled, the student and parents are required to attend.
